Perhaps this item is both a match holder and toothpick, many of these sort of items were made in mind to use for either/or.
I've not found this item in a toothpick holder book as of yet, but it is pictured in "Collectable Match Holders ~ for table and walls".
Authors Jean & Franklin Hunting ~ by Schiffer publishing ~ 1998. Page 52.
It's made completely of white metal (pot metal).
The authors feel this item was made approx circa 1900 - 1925.
The holder is 4 3/4" long ~ 2 3/4" tall.

There should be two sharp prongs on the birds peak which appear to be hidden by the center bar in these photos.
The prongs spear a match/toothpick when the Woodpecker's head is pressed down and the spring under the bird
brings the Woodpecker up-right after use.

The white spot on the under side is an opening in the base molded during manufacturing.
The copy I have in my match holder collection has the same opening.
